Office Hours
Patients are seen by appointment only. To schedule an appointment, call our main office at (608) 257-2208. Our office hours are 8AM to 4PM, Monday through Friday. We attempt to schedule our appointments at intervals to maintain a relaxed atmosphere and to minimize waiting. If you anticipate being late for your appointment, please call as soon as possible and we will be happy to accommodate you!
Emergencies
In the event that you require emergency care or have urgent problems, our staff can be reached 24 hours a day by calling our office (608)257-2208. During off hours, an answering service will provide you with the system to contact the particular staff member on call. One of our staff members is always available for you 24 hours each day.
Insurance
We are not affiliated with any of the major HMO's in the area. We do work with several independent insurance companies.
Payment
We do require that payment is made in full three weeks prior to the date of a patient's surgical procedure. We accept VISA, Master Card, American Express and Discover. We also accept personal checks, cashiers checks and cash.
Out of Town Patients
At Madison Plastic Surgery, we take care of patients from all over the United States. We have negotiated special rates for our patients at the local hotels and suites listed below. We recommend that patients who live farther than 20 to 30 minutes from our facility stay overnight for a day or two. Nursing assistance can be arranged during your stay, if you wish.

The following form can be downloaded to your computer and printed at home. By filling out this form ahead of time, you will save a significant amount of time during your visit. Please be prepared to provide this information to our office along with your current medical insurance information. If you have any questions when filling out this form, please do the best you can and our staff will assist you with your questions on the day of your visit.
